Healthcare Interoperability

For clinical decision support systems to be able to reason over disparate sources of data, there must be healthcare interoperability. To say that two systems are interoperable means they are capable of coordinating run-time behavior and exchanging data using agreed transport protocols, payload specifications, information models and workflows.

The Health Level Seven (HL7) community recognizes three levels of healthcare interoperability:

  • Technical interoperability where there is an agreed-to method of moving the data between systems (e.g., HL7 V3 results messaging between a laboratory machine and the EHR).
  • Semantic interoperability where systems are able to understand the information that is being exchanged (e.g., a medication list from the EMR is reliably analyzed by a Clinical Decision Support system for drug-drug or drug-allergy incompatibilities.
  • Process interoperability where a workflow can execute across systems (e.g., an ordering system that can execute transactions across a variety of fulfillment systems).
